We've expanded our service area!

While iCare Family Care Partnership continue to provide high quality service in Dane, Kenosha, Milwaukee, Racine, and Sauk Counties, we are excited to offer services to the following counties in 2025: Adams, Columbia, Dodge, Green Lake, Jefferson, Marquette, Rock, and Waushara Counties.

 

Am I eligible to join Family Care Partnership?

You are eligible if you:

  • Are an adult with a physical or developmental disability or are 60 years of age or older
  • Live in our service area for this plan, which includes these counties in Wisconsin:
    • Adams
    • Columbia
    • Dane
    • Dodge
    • Green Lake
    • Jeffersion
    • Kenosha
    • Marquette
    • Milwaukee
    • Racine
    • Rock
    • Sauk
    • Waushara
  • Are financially eligible for Wisconsin Medicaid
  • Are functionally eligible as determined by the State of Wisconsin Long-Term Care Functional Screen
  • You must be enrolled in the Family Care Partnership program with iCare as your Managed Care Organization (MCO)
  • If you are also Medicare eligible, you must be enrolled in Medicare Part A and be enrolled in Medicare Part B
