On January 1, 2019, our plan name will change from Aurora Special Needs Plan to AuroraCompleteCare. The health plan you love has a new name, but most things haven’t changed. You can still use the same doctors, your care team is still the same, and 2019 benefits are better than ever!

Building upon Aurora Health Care’s history of providing high-quality care and Independent Care Health Plan’s (iCare's) model of care, the Aurora Special Needs Plan (HMO SNP) will help you manage all of your benefits, so that you get the health care services and payment assistance that you are entitled to. Our partnership with Aurora Health Care will provide you with the best possible care through Aurora’s integrated health system of facilities, services and providers, including 15 hospitals, 159 clinics, 70 pharmacies and 32,000 amazing caregivers. Independent Care Health Plan (iCare) is a Medicare Advantage (HMO SNP) organization with a Medicare contract and a contract with the State Medicaid program. Enrollment in plans insured by iCare depends on contract renewal. Plans insured by iCare are available to anyone who has both Medical Assistance from the State and Medicare.
