    SilverSneakers is a fitness benefit for iCare Family Care Partnership members that have Medicare. This benefit provides access to 11,000+ fitness locations* nationwide, exercise equipment and other amenities, group exercise classes led by certified instructors, a support network and online resources. Signature SilverSneakers classes designed for all levels and abilities are offered in traditional fitness classrooms.

    More than 70 SilverSneakers FLEX© class options including Latin dance, yoga, tai chi and walking groups are offered in settings outside the traditional gym. Three BOOM© classes, MIND, MUSCLE and MOVE IT, offer more intense workouts inside the gym. Eligible plan members simply show their personal SilverSneakers ID number at the front desk of any participating location to use the benefit. Members may get their SilverSneakers ID number and find locations and classes by calling SilverSneakers at 1-888-423-4632.

    *At-home kits are offered for members who want to start working out at home or for those who can’t get to a fitness location due to injury, illness or being homebound.

  • DentistDental Benefits

    An extra $2,500 a calendar year for preventive and comprehensive dental services from our network providers

    • Oral Exams and Cleaning: Up to 2 per calendar year
    • Dental X-Rays: Up to 1 per calendar year
      • X-Rays are limited to either 1 panoramic or 1 full set per calendar year
    • Emergency Office Visits & Diagnostic Services: Up to 2 per calendar year
    • Simple Restorative Services
      • Limited to Amalgams/Resins (No root canals or crowns): One restoration per tooth per calendar year
    • Simple Extractions (No Surgical Extractions)
    • Basic Partials and Basic Dentures are covered, no coverage for repair

    Receive an EXTRA $150 Per Calendar Year Towards Lenses and Frames

    As an iCare Family Care Partnership member, you receive up to $150 a calendar year for lenses and frames, on top of the standard vision benefits provided by Medicaid and Medicare. This can be applied to any frames and lenses from our network of vision providers.

    Vision Care Benefits from Herslof Optical

    iCare and Herslof Optical Company are working to provide you with the ultimate in quality vision care. As an iCare Family Care Partnership member Herslof offers you:

    • One routine eye exam per 12 month period
    • One pair of eyewear from a selection of standard frames and lenses covered in full, once during a 12 month period
    • You may choose a frame from outside of the covered in full selection and receive a $25.00 credit towards that upgrade
    • A replacement pair of eyewear if you lose or break your original pair

  • TelehealthTeleHealth Benefit

    We are inviting you to try out a new exciting benefit for iCare Family Care Partner­ship members, where you can video chat with a provider from the comfort of your home or from a computer located at iCare’s Milwaukee office.

    If you have high-speed Internet access you can connect to an Urgent Care Provider, a Behavioral Health Specialist, a Nutritionist, or Nurse Practitioner via a live, two-way video through your home computer or smart phone using Amwell’s telehealth application. You can have a two-way video conference with a provider 24 hours-a-day, 7 days-a-week. Urgent care can be accessed on demand, while behavioral health and nutrition ser­vices require a scheduled appointment. Members receive a maximum of 12 visits per calendar year.

  • Nurse Advice24-Hour Nurse Advice Line

    Staying healthy starts by asking questions and getting up-todate information. When you need answers the iCare Nurse Advice Line will be there for you 24 hours-a-day/7 days a week.

    The Nurse Advice Line can give you peace of mind and help when you:

    • Wonder whether you need to make an appointment
    • Need information about medications, medical tests, or procedures
    • Want to find ways to talk more effectively with your healthcare provider
    • Want to learn how to take care of a new or chronic condition
    • Have questions about how you or your family can stay healthy

    The Nurse Advice Line service also provides an Audio Health Library that gives you access to hundreds of prerecorded messages on health care topics ranging from the common cold to heart disease. Each message is about three to five minutes long.

The iCare Family Care Partnership (HMO SNP) is a Coordinated Care plan with a Medicare contract and a contract with the State Medicaid program. Enrollment in iCare Family Care Partnership depends on contract renewal. This plan is available to anyone who has both Medical Assistance from the State and Medicare and functionally eligible as determined by the State of Wisconsin Long-Term Care Functional Screen. Benefits, premiums and/or co-payments/co-insurance may change on January 1 of each year. You must continue to pay your Medicare Part B premium. The Part B premium is covered by the State if you are a full-dual member. Premiums, co-pays, co-insurance, and deductibles may vary based on the level of Extra Help you receive. You must receive all routine care from plan providers.
